2011 Nomination Form

Annually, the Ohio University Alumni Association Board of Directors selects recipients for five different award categories, to recognize the numerous achievements and contributions alumni and friends make to their communities and Ohio University.

To make a nomination, please review the brief objective for each of the different awards and complete the below Nomination Form. You may also make your nomination by mail; send your responses to Alumni Awards, Konneker Alumni Center, Ohio University, 52 University Terrace, Athens, OH 45701. Nominations are accepted throughout the year. The deadline for the 2011 Awards is December 1, 2010.

For fullest consideration, each nomination must include a completed nomination form, two letters of support and a resume of the person being nominated.
